Peru's government recognized traditional ayahuasca use as National Cultural Heritage in 2008, yet it licenses neither centers nor shamans. Choosing well is entirely on you.

Community-mentioned centers rest on repeated first-hand Reddit reports, which is weaker evidence, and we treat it that way. One more honesty note before the list. Review footprints in this niche are small and unaudited. A 5.0 across 29 reviews and a 4.8 across 94 are different kinds of evidence, so we give the numbers with their sources and let you weigh them.

Best small center: AyaSelva

Our overall pick for a first or second retreat is AyaSelva, a family-run center in Tamshiyacu near Iquitos that caps every retreat at six guests. The 7-day program runs 4 ceremonies for $1,200 all-inclusive; the 12-day runs 7 ceremonies for $1,800. Both include a private room with its own bathroom, meals within the diet, and transport from Iquitos.

The ceremonies are led by Arnaldo, a curandero raised in Tamshiyacu in the Mestizo tradition, and the brew is cooked on site from vine and chacruna only. Guests can watch it being made. Screening happens before booking: a confidential health questionnaire plus a personal intake call, with a published contraindication list covering SSRIs, MAOIs, antipsychotics, heart disease, epilepsy, psychosis history and pregnancy.

Its screening is the strictest we found documented anywhere: a medical and psychological questionnaire developed with ICEERS, reviewed by a licensed doctor and a psychologist before you are accepted. One Reddit user put it plainly: "From everyone I have spoken to throughout Peru, this is one of the most carefully and professionally guided centers."

The trade-offs are scale and price. Groups run around 20 people, and the flagship 12-day retreat costs $4,440. For some people a large, highly structured container is exactly right. If you are the type who would feel lost in a group that size, look at the small centers instead.

Best medically framed: Etnikas

For readers whose main worry is what happens if something goes wrong medically, Etnikas in San Salvador near Cusco is the name that keeps coming up. Reddit users recommend it specifically for its clinical approach, citing a full staff that includes nurses. Its listed pricing starts around $1,100 for 5 days.

Our verification of Etnikas is shallower than for the Iquitos centers: we confirmed the operation and the way people talk about it, but ceremony counts and lineage details were not independently checked in this pass. It also sits in the Sacred Valley, which means altitude around Cusco's 3,400 meters, a real consideration if you have heart or blood pressure concerns. Flower of Life near Iquitos is woman-owned, keeps groups to about six, and carries a 4.9 across 106 Google reviews. What sets it apart in the research is how consistently women who traveled alone vouch for it. One wrote: "As a woman who traveled there alone, I felt completely safe the entire time. Highly recommend!"

That kind of first-hand testimony matters because the risk it answers is documented and real; the BBC and others have reported misconduct at Amazon retreats generally. It sits in our community-mentioned tier, so treat the praise as strong signal rather than settled fact.
